  • Part time or full time, temporary position until June 2022
  • $86K+ per annum (pro rata for part time) + Superannuation


About The Role

As an Assessment Officer you will undertake holistic Home Support Assessments as part of the Regional Assessment Service for the Commonwealth Home Support Programme (CHSP).

Undertake assessments using the NSAF which include: assessment, referral, care planning and care coordination.

You will focus on promoting consumer directed care to maximise independence, quality of life focusing on reablement and linking support to create a plan that will underpin each client's individual goals.

We are offering either a part time (minimum 30.4 hours per week) or full time position with the flexibility of working days and start and finishing times.

Please note that applicants must be willing to undergo a Police records check for the purposes of employment.

About You

The key thing for us is that you are genuinely interested in working with older people and have a passion about enabling active, independent living across the local community

You will be proactive, diligent and solutions oriented with exceptional decision-making skills and attention to detail.

Skills And Experience
  • Tertiary Qualifications in the behavioural sciences or a health related discipline.
  • Completed Statement of Attainment (SOA2) training (desirable or preferred)
  • Experience in comprehensive client assessments and service planning in the context of consumer directed care.
  • An understanding of the Commonwealth Home Support Programme
  • Excellent organisational and time management skills
  • Experience in providing outstanding customer service to people from diverse backgrounds and cultures.
  • Demonstrated competency with My Aged Care systems, its associated applications and procedures.
