We’re looking for a Community Key Worker with relevant experience who wants to join our 2,000+ team of staff and volunteers to make a difference on a full-time basis, working 38 per week based in Frankston

The Community Key Worker will provide CHSP clients and their families support and advice to ensure clients maintain independence, family supports and connection to the community.

You will be advising on referral services available and following up with clients to provide support for clients as their needs change to allow them to continue to live independently.

To be considered for the role and be successful in it, we’re looking for an who has the following key skills and experiences:

  • Minimum of Certificate IV in Aged Care – or equivalent
  • Extensive experience in an aged Care setting
  • Outstanding knowledge of the CHSP funding and operation models
  • Outstanding time management and organisational skills
  • Excellent communication skills, ability to liaise with a diverse range of stakeholders
  • Ability to contribute to team outcomes ‘one team’ approach
  • Excellent administration, computer skills and competency in Microsoft Program

 Salary will be in the range of $64,624,42 - $69,287.36 per annum, based on skills and experience plus superannuation
