About the role

The Youth Transitions Program works with our clients leaving care and supports their voices to be heard in their own transition planning. The support will begin in care and post care and span such areas as housing, health & wellbeing, education, employment, community and cultural connections.

Qualifications

Social Work, Welfare Studies or their equivalent or other tertiary qualification appropriate to Family and Community Services.

You are an enthusiastic and committed individual with demonstrated skills and experience engaging with young people and enjoy working collaboratively to achieve outcomes. Your highly developed cultural awareness and sensitivity, partnered with excellent communication skills, will enable you to quickly build strong relationships with clients and key stakeholders. You are not afraid of a challenge and constantly seek to grow and develop.
