Albury Wodonga Health (AWH) is a unique cross-border public health service and the largest regional health service between Sydney and Melbourne. The Opportunity

The position is responsible for providing the co-ordination of the day to day running of the Palliative Care Regional and Community Services, do that delivery of high quality patient centred care can be achieved.

Primary responsibilities of this role includes the delivery of an integrated, client service which provides health care and support to the patient either on hospital or at home and help prevent the need for a hospital stay/ presentation.
